Sports Physical Therapy Residency Program

University of EvansvilleFlyer Application Process

The applicant will submit the following information:

  • Cover letter (less than 2 pages) describing:
    • the reasons for your interest in participating in the residency
    • your long term plans in physical therapy
    • your orthopaedic or sports physical therapy experience
    • anticipated graduation date and physical therapy licensure exam completion
  • Resume or curriculum vitae that includes clinical experience and reference contact information.
  • Two completed recommendation forms which include at least one person who has supervised you in the clinic.
  • Documentation of one of the following: a current ATC designation, a current license
    as an EMT, or certification as an Emergency Responder through the APTA emergency
    response course or the EIM emergency response course.
  • A completed ProRehab application for employment.Application

Early application deadline January 1st. Application deadline March 15th.

Please mail the above application requirements along with a $50 non-refundable application fee (check payable to ProRehab, PC) to:

After review of you application, you may be asked to complete an interview.
