Q.
Do I need a prescription for physical and/or
occupational therapy?
A.
Physical &
Occupational Therapy performed in the
state of Indiana requires a referral/prescription
from a licensed physician.
Q. What should I bring for my first visit?
A. You should bring:
- The Physician Referral
Form filled out and signed by your doctor
- Notes, reports, x-rays or other test results from your doctor
- Insurance Information
- Braces or prosthesis
- Completed Patient Registration
and Medical History Form
(optional)
Q. What can I
expect at my first visit?
A.
On
your initial visit, the therapist will examine the
problem area and develop an effective treatment
plan. The patient will be asked various
questions
regarding how the injury took place, and what
types of symptoms are present. Any
additional
questions that you may have will also be answered
in detail at this time.
Q. Should I bring a list of my
medications?
A.
A
complete list of medications you are taking is
needed for your patient record. Medication
name,
dosage, and reason you are taking it is
helpful. It
is important your therapist have this information
in
order to modify, defer or add appropriate treatment
techniques that may be affected by certain
medications.
Q.
What should I wear to my therapy session?
A.
You should wear loose fitting, comfortable clothing
and tennis shoes. If you are coming for
a knee,
leg, ankle, or foot problem, shorts are
recommended. You can bring clothing
with you if
you need to change upon arrival Your therapist
will tell you if different clothing is
required.
Q.
How long will the visit take?
A.
The
initial examination will take 30-60
minutes.
Q. Will my therapist communicate
with my doctor
regarding my treatment?
A. Your therapist will send
progress notes to your
physician, depending on the length of your
treatment, either weekly or before you return to
your physician.
Q. What if I have to cancel an appointment?
A. Simply
call the clinic where your appointment is
scheduled and notify the front desk. Please
try to
give us 24 hours notice so that another patient
can
use your appointment time.
Q.
What do I do if I re-injure myself?
A. If you have a medical emergency, please
contact your
physician or go to an emergency
room.
